STAR behavioral interview questions: handling mistakes and failures

These questions follow the STAR (Situation, Task, Action, Result) framework and are designed to assess how candidates reflect on their mistakes, take ownership of outcomes, and deal with the consequences of failure.

  • Tell us about a time when you made a serious mistake at work. What was the situation and task, what went wrong, what did you do to fix it, and what was the outcome?
  • Describe a situation when you realized you were falling behind on an important task. What was happening, what did you do, and what was the result?
  • Tell us about a time when a project or initiative you were involved in failed completely. What did you learn from analyzing what went wrong?
  • Tell us about a time when you took responsibility for a failure that involved the entire team. How did you handle the situation?
  • Describe a situation when you had to fix a mistake made by a previous employee or a colleague.
  • Tell us about a time when you made the wrong decision because you were under time pressure. What were the consequences, and how did you change your approach afterward?
  • Recall a situation when you misunderstood a brief or requirements from a client or manager and delivered something different from what was expected. How did you resolve the issue?
  • Tell us about a time when you had to publicly acknowledge a mistake in front of your entire team or a client.
  • Tell us about a time when a technical or system failure happened due to something you did. How did you minimize the impact on the business?
  • Describe a situation when you spotted a colleague’s mistake that could have put a project at risk. What did you do, following the STAR approach?
  • Tell us about a time when you discovered a mistake after the project had already been delivered to the client.
